When you have a patient with dense cataract you are facing multiple challenges performing eye examination.

Optical biometers will not be able to provide an AXL, Slit lamp examination could be limited as well...

The only solution to complete eye examination is to use ophthalmic ultrasound.

Ultrasound biometer will allow you to measure AXL even is cataract is too dense.

B-scan will provide detailed image of the back of the eye

UBM will provide detailed image of anterior segment including behind the iris detailed view of ciliary body and zonules
